INSURANCE
While we do not accept insurance assignment, we are happy to assist you with the paperwork your insurance carrier may request for reimbursement. For your convenience we accept cash, personal checks and all major credit cards. All payment is expected at the time of service.

PAYMENT INFORMATION
Eastside Diagnostic Imaging, PLLC does not accept insurance assignment.  However, we will provide you with all the necessary paperwork and reports required to submit your claim to your insurance carrier or for those using a Health Savings Account (HSA) or “out of network benefits”, your benefits administrator.  For your convenience we accept cash, personal checks and all major credit cards. All payment is expected at the time of service.
